We are happy to register new NHS patients within our practice boundary area.
How to Register
To register with our practice you will be required to complete two registration forms per person: one is the standard NHS GP registration form (GMS1) and the second is a new patient practice registration form. Please use the appropriate forms from the links below:

After you have completed the forms you must attend the surgery within 7 days with the completed forms and two forms of identification. Alternatively you can email the completed Online form to firstname.lastname@example.org. Please ensure that you have checked your postcode is within our boundary. Due to the current Coronavirus outbreak, you will be asked to bring in your ID to your first appointment at the surgery.
Acceptable identification includes:
- Birth Certificate
- Driving licence
- Recent utility bill
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration/mail forwarding letter
All documents must be dated within the last 3 months.
We are unable to accept mobile phone bills as proof of residence.
We will need your NHS number when you register. Your previous GP surgery can provide you with this.
Until we have seen your identification you will not be registered at the practice.
